Case Management Supervisor, Personal Care Services

Full-Time | On-Site | Broken Arrow, OK

We are seeking a Case Management Supervisor to lead and support our case management team—ensuring exceptional care coordination, compliance, and service delivery for our members.

What You’ll Do

  • Lead, coach, and develop a team of Case Managers

  • Oversee client assessments, service planning, and care coordination

  • Ensure quality, compliance, and accurate documentation

  • Monitor team productivity, performance, and outcomes

  • Conduct home visits and support care plan implementation

  • Review member charts and ensure readiness for billing and authorization

  • Partner with leadership to improve processes and drive outcomes

  • Support recruitment, training, and onboarding of team members

  • Build relationships with clients, families, and referral sources

What We’re Looking For

  • Active RN or LPN license (required)

  • Bachelor’s degree preferred (or equivalent experience)

  • 2+ years of experience working with elderly or disabled populations

  • Leadership or supervisory experience preferred

  • Strong communication, organization, and team leadership skills

  • Knowledge of home care, case management, and compliance standards

  • Ability to manage performance, productivity, and quality metrics

  • Reliable personal transportation (travel required ~15%)
